If you have experienced any issues that mean you have not been able to complete your examination in a straightforward manner, you should complete an Online Examinations Incident Report.

This may not apply to in-person examinations. 

How do I report incident in an online examination

  1. Complete the Online Examinations Incident Report for each paper with which you have experienced issues.
  2. This report should also be used if you have any questions about the content of the online examination itself.
  3. To report an incident that you experienced during an online examination, complete the Online Examinations Incident Report:
