You should report health and safety incidents or hazards, to identify risks within the workplace and create controls.
How do I report an incident or hazard
To log an incident in Vault:
- Open Vault.
- Click the appropriate tab for the incident you are reporting.
- Fill in all the boxes marked with an * and provide as much detail as possible.
- Once the form is completed, click Save.
- You will be given a reference number to save or write down for future reference.
- An email will be sent to the supervisor of the person involved in the incident and/or the departmental health and safety officer.
