An incident report form (for in-person examinations) or online examination incident report form (for online examinations) may be used to report any issues experienced during a final examination.

Issues may include:

  • Problems with the examination experience that affect your performance, including technical difficulties, illness, or other exceptional circumstances
  • Issues with the content of the examination
  • IT or system-related problems that occur during an online examination

In-person examination incident report forms are available from examination supervisors at the time of the examination. Examination supervisors can complete an incident report form on behalf of a student or provide a copy to a student who wishes to complete their own.

Online examination incident report forms are submitted online. These should be submitted if you experience a problem during an online examination that affects your performance, such as an IT or technical issue, illness, or other exceptional circumstances. When submitting an online incident report, describe the issue and any actions taken in as much detail as possible.

Staff must also complete the form when assisting students with an examination incident.
