An incident report form (for in-person examinations) or online examination incident report form (for online examinations) may be used to report any issues experienced during a final examination.
Issues may include:
- Problems with the examination experience that affect your performance, including technical difficulties, illness, or other exceptional circumstances
- Issues with the content of the examination
In-person examination incident report forms are available from examination supervisors at the time of the examination. Examination supervisors can complete an incident report form on behalf of a student or provide a copy to a student who wishes to complete their own.
Online examination incident report forms are submitted online - log an online examination incident report (login required).
